How we decide pass / flag / block
- 01NPDB queries require provider consent on file (signed within 60 days).
- 02Any report is surfaced verbatim — UPP does not summarize or interpret.
- 03Self-Query results from the provider are accepted when issued within 90 days.
- 04Continuous Query enrollment, when active, replaces scheduled re-query.
